Sarah Schaefer, MSN, RN, ANP-BC

Founder, Independent Patient Advocate

With more than 25 years in healthcare, I have always found that one part of my work stood out above the rest: advocating for my patients.

I loved helping people understand what was happening, making sure their questions were answered, helping them speak up when something didn't feel right, and making sure their needs didn't get lost in a complicated healthcare system.

Over the years, I realized that advocacy was not just one part of my nursing career—it was the part I cared about most.

That is what led me to make the decision to focus on patient advocacy full time and found Ascent Patient Advocates.

As a nurse practitioner, I bring a strong clinical background and years of experience with diagnoses, treatments, medications, medical records, and the healthcare system. I understand the language of healthcare and the many moving parts that can make it difficult for patients and families to know what to do next.

But advocacy is different from providing medical care.

My job is not to tell you what decision to make. My job is to help you understand your options, prepare for important conversations, organize information, and make sure your voice is heard.

As an independent patient advocate, I work directly for you. I am not employed by a hospital, insurance company, or healthcare system. That independence allows me to keep my focus where it belongs: on you, your needs, your priorities, and your goals.

Sometimes that means preparing for an important medical appointment. Sometimes it means helping an adult child coordinate care for an aging parent. Sometimes it means making sense of medical records, medications, test results, or a new diagnosis. And sometimes, it simply means having someone knowledgeable in your corner who can help you figure out what comes next.

I provide personalized healthcare navigation and care coordination for adults and older adults throughout Wisconsin, as well as nationwide support by phone or video.

You should not have to become an expert in healthcare to be an informed participant in your own care. I'm here to help.

A little more about me

Outside of work, I enjoy cheering on the Badgers, spending time with family and our dog, and getting outdoors whenever I can.

I bring the same qualities to my work that I value in everyday life: curiosity, compassion, honesty, and a genuine desire to help people move forward.

For me, patient advocacy is more than a profession. It is the part of healthcare I have always loved most.
